Please do not include detailed medical information in the website form. We will discuss your health history in person, at your consultation.

We do not sell, rent or trade your personal information, and we do not send you marketing messages unless you ask us to.
Health information collected at the clinic
Medical history, consent forms, treatment notes and photographs taken at the clinic are personal health information. They are collected with your consent, used only to provide and document your care, and kept confidential in accordance with Ontario’s Personal Health Information Protection Act (PHIPA) and the College of Nurses of Ontario practice standards. Treatment photographs are used on this website or in any promotion only with your separate written consent, which you may withdraw at any time.
The downloadable consent and medical history forms on this website are blank. Once filled in, please bring them with you to your appointment rather than sending them by email.
